Your Pregnancy Your Choices Brochure This brochure has important information about pregnancy choices and MUST be available to all Title X clients who present with a positive pregnancy test, unless upon request, the pregnant client indicates she does not wish to receive such information. If the pregnancy choices brochure is declined by the pregnant client, this information MUST be documented in the client's chart.
